Gemfibrozil/Lopid

Lopid belongs to a group of medications commonly known as fibrates. These medicines are used to reduce the level of fats (lipids) in the blood. For example, fats called triglycerides. The indications for Lopid is to supplement to diet and non-pharmacological management (such as exercise, weight loss) in the following cases: Treatment of severe hypertriglyceridemia with or without low HDL cholesterol. Lopid capsules can be used in the treatment of hypercholesterolemia as a replacement or complementary therapy to statins.

The main active ingredient in the Lopid capsules, Gemfibrozil, inhibits peripheral lipolysis by decreasing the hepatic extraction of free fatty acids, which, in turn, decreases the hepatic production of triglycerides. Gemfibrozil also inhibits the synthesis and increases levels of apolipoprotein B, a VLDL carrier molecule. Gemfibrozil can also speed up the removal of cholesterol from the liver and increase the excretion of cholesterol in the stool.

The effects of gemfibrozil on triglyceride and HDL fraction concentrations are greater than those of clofibrate. Gemfibrozil has variable effects on LDL cholesterol. Although it causes a moderate reduction in patients with type IIa hyperlipoproteinemia, the changes in patients with type IIb hyperlipoproteinemia or type IV hyperlipoproteinemia are unpredictable. In general, HMG-CoA reductase inhibitors are more effective than gemfibrozil in lowering LDL cholesterol.

Lopid is used, along with a low-fat diet and other non-medical treatments such as exercise and weight loss, to reduce fat levels in the blood. Lopid can be used when other medications are not suitable for reducing the occurrence of heart problems in men who are at high risk and whose "bad cholesterol" is higher. Lopid may also be prescribed to lower blood cholesterol levels for those who cannot be prescribed other lipid-lowering medications.

Always use this medicine exactly as your doctor or pharmacist has told you. Ask your doctor or pharmacist if you have doubts.

You will be closely and regularly monitored for blood lipid levels before and during Lopid treatment. If you are diabetic or have thyroid problems, your doctor will have to treat these conditions before treatment begins. Your doctor will also advise you on the diet to follow, exercise, quitting smoking, reducing alcohol consumption, and, if necessary, losing weight.

Lopid must be taken orally. It is recommended to swallow the tablets with a glass of water

Like all medicines, this medicine can have side effects, although not everybody gets them.

Although serious reactions may occur, you should contact your doctor immediately if you experience any of the following symptoms after taking Lopid:

Rare (may affect up to 1 in 1,000 people)

  • allergic reaction in which the face, tongue or throat can become inflamed, causing difficulty breathing (angioedema)
  • peeling and blisters on the skin, mouth, eyes, and genitals
  • rash that affects the entire body
  • muscle weakness or weakness accompanied by dark urine, fever, fast heart rate (palpitations), nausea, vomiting

  • indigestion

  • irregular heartbeat

Rare (may affect up to 1 in 1,000 people)

  • reduction or increase of white blood cells (leukopenia, eosinophilia), bone marrow disease (bone marrow failure)
  • reduced platelet count (thrombocytopenia)
  • inflammation of the nerves (peripheral neuritis)
  • unusual bruising or bleeding from reduced blood platelets (thrombocytopenia)
  • severe anemia
  • loss of feeling and tingling sensation (paresthesia)
  • pancreatitis
  • blurry vision
  • jaundice (yellowing of the skin), abnormal liver function
  • inflammation of the liver (hepatitis)
  • gallstones (cholelithiasis), inflammation of the gallbladder (cholecystitis)
  • appendicitis
  • depression
  • dizziness
  • drowsiness
  • joint and limb pain
  • skin inflammation or inflamed skin that flakes or falls off
  • inflammation of the muscles (myositis)
  • inflammation of the synovial membrane (synovitis)
  • persistent lack of energy
  • impotence
  • reduced libido
  • loss of hair
  • photosensitivity (sensitivity to light that can cause skin color change or rash)
  • raised, itchy red areas of skin
  • itchiness
