Colestid

Colestid granules are a prescription-only bile acid sequestrant containing colestipol hydrochloride, used in adults as adjunctive therapy to diet for the reduction of elevated LDL cholesterol. Colestid Granules for Oral Suspension is a prescription medicine manufactured by Pfizer. The active ingredient is colestipol hydrochloride. Colestipol belongs to the bile acid sequestrant class of lipid-lowering medicines, also known as anion exchange resins. This is the same class as colesevelam (Cholestagel) and colestyramine, though each product has different dosing requirements and tolerability profiles.

Colestid granules are licensed in the UK as adjunctive therapy to diet for the reduction of elevated serum total cholesterol and LDL cholesterol in patients with primary hypercholesterolaemia who do not respond adequately to diet alone. Colestid may be used as monotherapy when a statin is not appropriate, or alongside a statin or other lipid lowering treatment for further LDL reduction. Because colestipol is not absorbed into the body, it has no systemic side effects such as muscle problems or liver enzyme changes, which are associated with statins.

Colestid granules come in 5g sachets. Each sachet is mixed with at least 90ml of water, juice, or another suitable liquid before swallowing. Colestid is also available as 1g tablets, which is a separate formulation and product. This page covers the granules (oral suspension) formulation only.

Colestid lowers LDL cholesterol through a mechanism that is entirely local within the digestive system. Colestipol is not absorbed and does not enter the bloodstream.

Bile Acid Sequestration. The liver converts cholesterol into bile acids, which are secreted into the small intestine to assist fat digestion. Normally, most bile acids are reabsorbed from the small intestine and returned to the liver through enterohepatic circulation. Colestipol is a large positively charged resin that binds to negatively charged bile acids in the gut. The resulting complexes cannot be absorbed and are excreted in the faeces, removing bile acids from the enterohepatic cycle.

LDL Receptor Upregulation. When the liver receives less recycled bile acid, it increases its own conversion of cholesterol into new bile acids. This increased internal demand causes liver cells to upregulate LDL receptors on their surface. These additional LDL receptors capture more LDL cholesterol from the bloodstream, reducing circulating LDL levels.

Because colestipol works locally in the gut and is never absorbed, it does not interact with medicines in the bloodstream. However, it can bind to other oral medicines in the gut and reduce their absorption if they are taken at the same time, making timing critically important.

Always take Colestid granules exactly as your prescriber instructs. Read the Patient Information Leaflet supplied with your medicine before starting treatment.

• Never take Colestid granules dry. Always mix the granules with liquid before swallowing.
• Mix each 5g sachet with at least 90ml (approximately a third of a pint) of water, fruit juice, skimmed milk, or another suitable liquid. Stir well — the granules will not dissolve completely, this is normal. Drink the mixture straight away.
• Rinse the glass with a small amount of additional liquid and drink that too.
• Colestid granules should be taken with meals or shortly before a meal, not on an empty stomach.
• Other oral medicines must be taken at least one hour before or four to six hours after Colestid granules.

If you miss a dose, take it as soon as you remember with your next meal. If you only remember between meals, skip the missed dose and take the next scheduled dose with your following meal. Do not take two doses at once. Do not stop taking Colestid without speaking to your prescriber first.

Your prescriber will arrange blood tests to monitor your lipid levels before and after starting treatment.

Each sachet contains 5g of colestipol hydrochloride. The SmPC recommends a starting dose of 5g (one sachet) once or twice daily, mixed with liquid as described above. The dose may be increased gradually in 5g increments at intervals of one to two months based on lipid response and tolerability. The usual maintenance dose ranges from 5g to 30g per day in one or two divided doses. Higher doses produce greater LDL reduction but also increase the risk of gastrointestinal side effects, particularly constipation.

Gradual dose titration is important because starting at high doses significantly increases gastrointestinal side effects. Taking the lowest effective dose and increasing slowly allows the bowel to adjust and reduces the burden of constipation. All dose adjustments are made by your prescriber. Do not change your dose without medical advice.

The side effects of Colestid granules are predominantly gastrointestinal and reflect its mechanism of action in the gut. Because it is not absorbed into the body, it does not cause systemic side effects.

Common Side Effects (may affect up to 1 in 10 people):

• Constipation — the most commonly reported side effect. Starting at the lowest dose and increasing gradually helps minimise this. Drinking at least eight glasses of water per day and maintaining adequate dietary fibre intake are strongly recommended.
• Nausea, bloating, and abdominal discomfort.
• Flatulence.

Uncommon Side Effects (may affect up to 1 in 100 people):

• Raised triglyceride levels, monitored during treatment.
• Haemorrhoids or perianal irritation, often related to constipation.
• Headache.

Serious Side Effects — Seek Urgent Medical Attention: Go to your nearest accident and emergency department or call 999 immediately if you experience choking or difficulty swallowing after taking the granules; signs of bowel obstruction such as severe abdominal pain, inability to pass stools, and vomiting; signs of fat-soluble vitamin deficiency during long term use, such as easy bruising or bleeding (vitamin K) or bone pain or muscle weakness (vitamin D); or signs of a severe allergic reaction such as sudden widespread rash, difficulty breathing, or swelling of the face, lips, tongue, or throat.

Colestid granules may be suitable if you:

• Are an adult with primary hypercholesterolaemia where LDL reduction is the treatment goal and dietary measures have been insufficient.
• Cannot tolerate a statin and require a statin alternative for cholesterol management as monotherapy.
• Are taking a statin and require further LDL reduction alongside your current treatment.
• Are able to mix the granules with liquid and drink them reliably at each dose.
• Are already prescribed Colestid granules and require a repeat prescription through an online prescribing service.

Colestid granules are not suitable if you:

• Have complete biliary obstruction.
• Have severely elevated triglyceride levels.
• Have dysphagia or oesophageal disorders.
• Have a history of bowel obstruction or severe gastrointestinal motility disorders.
• Are pregnant or planning to become pregnant.
• Are breastfeeding.
• Are under 18 years of age.
• Have a known allergy or hypersensitivity to colestipol hydrochloride or any excipient listed in the SmPC.

Important Drug Interactions — Timing Is Critical: All other oral medicines must be taken at least one hour before or four to six hours after your Colestid dose. This includes statins, thyroid medicines such as levothyroxine, warfarin and other anticoagulants, fat-soluble vitamins (A, D, E, K) and folic acid, digoxin, thiazide diuretics, and many other medicines. Always inform your prescriber of all medicines you take before starting Colestid.
