How it works.
Dexamethasone
The exact mechanism of dexamethasone's anti-inflammatory action is unknown. It inhibits multiple inflammatory cytokines and produces multiple glucocorticoid and mineralocorticoid effects.
Polymyxin B
It is a cyclic lipopeptide that penetrates the cell wall of Gram-negative bacilli and thus destabilizes its cytoplasmic membrane. It is generally less active against Gram-positive bacteria.
Neomycin
It primarily works by inhibiting protein synthesis in the ribosomes of bacterial cells by inhibiting the assembly of the polypeptide.
Resistance mechanism
Bacterial resistance to polymyxin B is of chromosomal origin and is rare. It appears to be due to a modification of the phospholipids of the cytoplasmic membrane. Neomycin resistance occurs through several different mechanisms: (1) alterations of the ribosomal subunit inside the bacterial cell, (2) interference with the transport of neomycin into the cell, and (3) inactivation by a series of enzymes capable of adenylation, phosphorylation, and acetylation. Genetic information for inactivating enzymes can be found on the bacterial chromosome or on plasmids.
Maxitrol eye drops are indicated in the treatment of infectious eye inflammations caused by organisms that are sensitive to Neomycin Sulfate and Polymyxin B Sulfate, These symptoms include Non-purulent conjunctivitis, acute or chronic blepharoconjunctivitis, and keratoconjunctivitis. In addition to Non-specific superficial keratitis, deep keratitis, acne rosacea keratitis. Iridocyclitis. Moderate acute arthritis, Recurrent marginal ulcer, Corneal ulcer, Non-purulent blepharitis, and Esceritis episcleritis.
Always use this eye swelling treatment medication exactly as your doctor or pharmacist has told you. If in doubt, consult your doctor or pharmacist again. The recommended dose is:
Use in adults (including elderly patients)
Instill 1 or 2 drops into the conjunctival sac 4 or 6 times a day. The dose may be reduced after 3 or 4 days if a satisfactory response has been obtained.
- Wash your hands.
- Take the bottle (dropper container).
- Shake well before using.
- After opening the bottle for the first time, you must remove the plastic ring from the seal if it is loose.
- Hold the bottle upside down between your fingers.
- Tilt your head back. Gently separate the eyelid from the eye with a finger until a pocket forms between the eyelid and your eye, into which the drop should fall.
- Bring the tip of the bottle close to the eye. A mirror may be helpful.
- Do not touch the eye or eyelid, proximal areas, or other surfaces with the dropper. The drops could become contaminated.
- Gently squeeze the base of the bottle with your index finger so that one drop falls at a time.
- After using this medicine, release the eyelid, close your eye and gently press your finger to the edge of the eye next to the nose. This helps prevent this medication from passing to the rest of the body.
- If drops are applied to both eyes, repeat all of the above steps with the other eye.
- Close the bottle tightly immediately after using the product.
If a drop falls out of the eye, try again.
Like all medicines, this medicine can cause side effects, although not everybody gets them.

Effects in the eye:
- inflammation of the surface of the eye, increased pressure in the eye, itchy eye, eye discomfort, eye irritation

Effects in the eye:
- corneal ulcer, blurred vision, sensitivity to light, increased size of the pupil, drooping of the eyelid, pain in the eye, swelling in the eye, abnormal sensation in the eye, redness of the eye, increased tear production.
General effects:
- allergy, headache, severe skin reactions (Stevens-Johnson syndrome).
Hormonal problems:
- Excessive growth of body hair (particularly in women), muscle weakness and wasting, purple striations on the skin of the body, increased blood pressure, irregular or absent menses, changes in the body's protein and calcium levels, growth retardation in children and adolescents and swelling and weight gain of the body and face (Cushing's syndrome)
