Fostair Nexthaler is a powder that is inhaled through your mouth and released deep into your lungs. It contains two active ingredients: beclomethasone dipropionate anhydrous and formoterol fumarate dihydrate.
Beclomethasone dipropionate belongs to a group of medications called corticosteroids that have an anti-inflammatory action by reducing the swelling and irritation of your lungs.
Formoterol fumarate dihydrate belongs to a group of medicines called long-acting bronchodilators that relax the muscles of the airways by causing them to dilate, making it easier for air to get in and out of the lungs.
Fostair Nexthaler contains beclomethasone dipropionate and formoterol formulated in dry powder resulting in an extra-fine aerosol with a mean aerodynamic diameter mass (MMAD) of 1.4-1.5 microns and co-deposition of both components. Fostair Nexthaler's aerosol particles are on average much smaller than the particles released in non-extra-fine formulations.
A study of radiolabelled drug deposition in adult asthmatic patients showed that a high proportion of drug (estimated 42% of the nominal dose) is deposited in the lung, with homogeneous deposition through the airways. These release characteristics confirm the use of low doses of corticosteroids with outstanding local pharmacodynamic effects, which were shown to be equivalent to the corresponding solution for inhalation in pressure packaging.
The two active ingredients in Fostair Nexthaler have different mechanisms of action. As with other combinations of corticosteroids and β2-agonists, additive effects are observed in terms of reducing asthma exacerbations.
Beclomethasone dipropionate
Beclomethasone dipropionate administered by inhalation at the recommended doses has a glucocorticoid anti-inflammatory action in the lungs that results in a reduction in symptoms and asthma exacerbations, with fewer adverse effects than when systemically administered corticosteroids.
Formoterol
Formoterol is a selective β2-adrenergic agonist that relaxes the bronchial smooth muscle in patients with reversible airway obstruction. The bronchodilator effect starts quickly (3 minutes after inhalation) and lasts up to 12 hours after dose administration.
Fostair Nexthaler is indicated for the routine treatment of asthma when the administration of a combination (a corticosteroid and a long-acting β2-agonist by inhalation) is appropriate:
- insufficiently controlled patients with inhaled corticosteroids and short-acting beta2-agonists administered "on-demand"
- adequately controlled patients with inhaled corticosteroids and long-acting beta2-agonists
Fostair Nexthaler is indicated in adult patients for the symptomatic treatment of patients with severe COPD and a history of repeated exacerbations, which present significant symptoms despite the usual treatment with long-acting bronchodilators.
Always use this medicine exactly as your doctor has told you. If in doubt, ask your doctor, nurse, or pharmacist again.
Fostair Nexthaler provides an extra-fine particle size distribution that results in a higher dose released in your lungs. Therefore, your doctor may prescribe a lower dose of this inhaler compared to your previous inhalers.
Asthma
You will be checked regularly by your doctor to make sure you are taking the optimal dose of Fostair Nexthaler. Your doctor will always adjust the treatment to the minimum dose that best controls your symptoms. In no case modify the dose without first consulting your doctor.
Fostair Nexthaler can be prescribed by your doctor in two different ways:
Use Fostair Nexthaler daily to treat your asthma along with a separate "rescue" inhaler to treat a sudden worsening of asthma symptoms such as shortness of breath, wheezing, and cough. Use Fostair Nexthaler daily to treat your asthma and also use Fostair Nexthaler to treat a sudden worsening of asthma symptoms such as shortness of breath, wheezing, and cough.
Using Fostair Nexthaler together with a separate "rescue" inhaler:
Adults and the elderly:
The recommended dose of this medicine is 1 or 2 inhalations twice a day.
The maximum daily dose is 4 pulses.
How to use your Nexthaler inhaler If you are not sure that you are receiving the dose correctly, contact your pharmacist or doctor. If you are not sure that the dose counter has a discounted one after your inhalation
Like all medicines, Fostair Nexthaler can cause side effects, although not everybody gets them.
As with other inhaler treatments, there is a risk of worsening shortness of breath, cough, and wheezing immediately after using Fostair Nexthaler, which is known as paradoxical bronchospasm. If this happens, STOP using Fostair Nexthaler and then use your quick-acting inhaler to treat symptoms. Contact your doctor immediately
Contact your doctor immediately if you experience hypersensitivity reactions, such as skin allergies, itching, rash, redness of the skin, swelling of the skin or mucosa, especially in the eyes, face, lips, and throat.
Other possible side effects of Fostair Nexthaler are listed below according to their frequency.
Common (at least 1 in 10 patients):
- tremors
- pneumonia (infection of the lungs) in patients with COPD.
Tell your doctor if you have any of the following symptoms while inhaling Fostair Nexthaler, they could be symptoms of a lung infection:
- Fever or chills
- Increased mucus production, change in mucus color
- Increased cough or increased difficulty breathing
Uncommon (at least 1 in 100 patients):
- flu-like symptoms, sore throat
- fungal infections (of the mouth and throat). To reduce these side effects, rinse your mouth or gargle with water or brush your teeth after using the inhaler.
- worsening asthma symptoms, shortness of breath
- hoarseness
- cough
- exceptionally fast heartbeat
- exceptionally slow heartbeat
- A feeling of tightness in the chest
- headache
- sickness
- feeling tired or snowy
- electrocardiographic abnormalities (ECG)
- decreased cortisol concentration in urine or blood
- An increased concentration of potassium in the blood
- increased blood glucose concentration
- An increased concentration of fatty acids in the blood.
Unknown frequency (frequency cannot be estimated from the available data):
- blurry vision.

- palpitations
- heart rhythm disorders
- disturbances or decreased sense of taste
- muscle pain and cramps
- restlessness, dizziness
