This heart medication increases coronary flow by selectively dilating the small resistance vessels that supply blood to the heart. This coronary vasodilation involves the accumulation of an endogenous compound, adenosine, which is a potent vasodilator and inhibitor of platelet aggregation. Adenosine induces vasodilation by directly stimulating adenosine receptors located on smooth muscle cell membranes and/or, indirectly, by increasing levels of cyclic AMP, an inhibitor of platelet function. Adenosine also interferes with the enzymatic degradation of cyclic AMP by phosphodiesterases. Dipyridamole probably inhibits adenosine deaminase and
phosphodiesterases, allowing cyclic AMP levels to remain elevated. The coronary vessels of the ischemic zone are not affected by dipyridamole, probably because they are already fully dilated.
The mechanism of action of dipyridamole as an inhibitor of platelet aggregation remains to be elucidated, although it is believed that the same mechanism that explains its vasodilatory effects may be involved. Dipyridamole-induced AMP-cyclic elevations can block the release of arachidonic acid from membrane phospholipids by reducing the activity of thromboxane A. In addition, dipyridamole directly stimulates the release of prostacyclin, which induces adenyl cyclase activity, increasing concentrations platelet cyclic AMP and thus further inhibiting platelet aggregation.
