What Is AQUACEL Ag+?
AQUACEL Ag+ is a regulated Class IIb medical device from Convatec. It is the next-generation version of AQUACEL Ag, retaining the ionic silver CMC hydrofiber technology of the original product and adding two further active agents: EDTA (ethylenediaminetetraacetic acid) and benzethonium chloride. These additions are specifically targeted at wound biofilm, which is a recognised barrier to healing in chronic wounds and is not effectively managed by silver alone.
Standard silver dressings including AQUACEL Ag are effective against planktonic bacteria, free-floating bacteria in the wound fluid and wound surface. Biofilm, however, is a structured community of microorganisms embedded in a self produced protective matrix called an extracellular polymeric substance. This matrix acts as a physical barrier that limits the penetration of silver ions and other antimicrobials to the bacteria within the biofilm. - Remove AQUACEL Ag+ from its sterile packaging immediately before use. Do not pre-wet.
Sheet Application
- Place the dry sheet over the wound, ensuring it covers the wound bed and extends slightly beyond the wound margin.
- Apply an appropriate secondary dressing to secure and provide additional absorbency as needed.
- Label with the date and time of application.
Ribbon Application for Cavity Wounds

Contraindications
- Known sensitivity or allergy to silver, EDTA, benzethonium chloride, CMC hydrofiber, or any component of the dressing.
- Dry or minimally exuding wounds - the dressing requires exudate to activate gelling.
- Concurrent use with oil-based topical products under the dressing, as these impair active agent release.
Precautions
- Single use only. Do not reuse or resterilise.
- Silver containing dressings may cause grey or blue grey discolouration (argyria) of the wound bed and periwound skin. Expected, localised, and reversible.
- For ribbon use: pack loosely, always leave a retrieval portion outside the wound, and count ribbons in and out at every dressing change.

AQUACEL Ag+ is a next-generation silver hydrofiber dressing from Convatec specifically designed to manage both wound infection and wound biofilm. It adds EDTA and benzethonium chloride to the established AQUACEL Ag ionic silver CMC hydrofiber technology, giving it enhanced biofilm-targeting capability. Both use ionic silver CMC hydrofiber technology with vertical wicking and cohesive gel properties. AQUACEL Ag+ adds EDTA and benzethonium chloride specifically to target wound biofilm. EDTA disrupts the biofilm matrix by chelating the structural calcium and magnesium ions. Benzethonium chloride adds antiseptic activity at the cellular level. Choose AQUACEL Ag+ when biofilm is a known or suspected barrier to healing. Choose standard AQUACEL Ag for straightforward wound infection management where biofilm is not a specific concern.
Wound biofilm is a community of microorganisms, typically bacteria, embedded in a self produced protective matrix called an extracellular polymeric substance. This matrix physically shields the bacteria from the immune system and from many antimicrobial agents including silver ions. Biofilm is estimated to be present in the majority of chronic non-healing wounds and is thought to be a significant reason why some chronic wounds fail to heal despite appropriate treatment. Disrupting the biofilm matrix before applying antimicrobial agents increases the effectiveness of wound infection management.
EDTA (ethylenediaminetetraacetic acid) is a chelating agent that binds to calcium and magnesium ions. Biofilm matrix requires these metal ions for its structural integrity. By chelating them, EDTA destabilises the extracellular polymeric substance matrix that surrounds and protects the biofilm community. This disruption makes the bacteria within the biofilm more accessible to the ionic silver and benzethonium chloride released alongside the EDTA. The combination of matrix disruption and antimicrobial activity provides a more comprehensive approach to biofilm management than silver alone.
Yes. AQUACEL Ag+ is available in a Ribbon format specifically for cavity wounds, sinus tracts, and tunnelling wounds where a flat sheet cannot reach the wound depth effectively. The 1cm x 45cm ribbon suits narrow cavities and sinus tracts. The 2cm x 45cm suits wider cavities. Introduce the ribbon loosely, always leave a retrieval portion outside the wound, apply a secondary dressing, and count ribbons in and out at every dressing change. The cohesive gel formed by the CMC fibres assists with complete retrieval of the ribbon at removal.
Choose AQUACEL Ag+ when biofilm is a known or suspected barrier to wound healing. Clinical signs suggesting biofilm include a chronic wound that fails to progress despite appropriate management, recurring wound infection despite silver dressing use, or a wound surface with a shiny or slimy appearance associated with biofilm. For wounds where infection is present but biofilm is not specifically suspected, standard AQUACEL Ag provides equivalent silver antimicrobial activity. If in doubt, seek clinical guidance from a tissue viability nurse.
AQUACEL Ag+ can be used at each dressing change for as long as infection or biofilm management is clinically indicated. Silver dressings are generally used for periods of two to four weeks and then reassessed for continued need. Prolonged silver dressing use without clinical reassessment is not recommended. Once the wound shows signs of progressing toward healing and infection or biofilm are no longer a concern, stepping down to a non-antimicrobial dressing is appropriate. Seek guidance from a tissue viability nurse or healthcare professional on the appropriate duration of use.
