What is Xigduo?
Xigduo contains two different active ingredients called dapagliflozin and metformin. Both belong to a group of medicines called "oral antidiabetics". These are medications taken by mouth for diabetes.

Xigduo combines two antihyperglycemic drugs with different and complementary mechanisms of action to improve glycemic control in patients with type 2 diabetes: dapagliflozin, an SGLT2 inhibitor, and metformin hydrochloride, a member of the biguanide class.
Dapagliflozin
Dapagliflozin is a very potent selective and reversible inhibitor (Ki: 0.55 nM) SGLT2.

Despite the presence of hyperglycemia in type 2 diabetes, reabsorption of filtered glucose continues. Dapagliflozin improves fasting and postprandial plasma glucose levels by reducing renal reabsorption of glucose, leading to the excretion of glucose in the urine. This glucose excretion (glucuronic effect) is observed after the first dose, is continuous during the 24-hour administration interval and is maintained during treatment. The amount of glucose eliminated by the kidney through this mechanism depends on the concentration of glucose in the blood and on GFR. Dapagliflozin does not alter normal endogenous glucose production in response to hypoglycemia. Dapagliflozin works independently of insulin secretion and the action of insulin. An improvement in the assessment of the homeostasis model for beta-cell function (HOMA in beta cells) has been observed in clinical studies with dapagliflozin.
Dapagliflozin-induced urinary glucose excretion (glucose) is associated with calorie loss and weight reduction. Inhibition of the sodium and glucose cotransporter by dapagliflozin was also associated with mild diuresis and transient natriuresis.
Dapagliflozin does not inhibit other glucose transporters important for glucose transport to peripheral tissues and is> 1,400 times more selective by SGLT2 against SGLT1, the main intestinal transporter responsible for glucose absorption.
Metformin
Metformin is a biguanide with antihyperglycemic effects that reduces basal and postprandial plasma glucose. It does not stimulate the secretion of insulin and, consequently, it does not produce hypoglycemia.
Metformin can act by three mechanisms:
- by decreased hepatic glucose production by inhibiting gluconeogenesis and glycogenolysis;
- slightly increasing insulin sensitivity, improving peripheral glucose uptake and utilization in muscle

Lactic acidosis occurs rarely (may affect up to 1 in 10,000 people)
Xigduo can cause a very rare but very serious side effect, called lactic acidosis. If this happens to you, you should stop taking Xigduo and contact a doctor or the nearest hospital immediately, since lactic acidosis can lead to coma.
Stop taking Xigduo and see a doctor as soon as possible if you notice any of the following serious or potentially serious side effects:
Dehydration: Excessive loss of body fluids, occurs infrequently (can affect up to 1 in 100 people).
These are the signs of dehydration:
- very dry or sticky mouth, feeling of intense thirst
- A feeling of drowsiness or tiredness
- Little or no urination (urine)
- fast heartbeat.
Urinary tract infection occurs frequently (can affect up to 1 in 10 people).
Here are the signs of a serious urinary tract infection:
- fever and/or chills
- burning sensation when urinating (urinating)
- back or side pain.
- Although rare, if you notice blood in your urine, tell your doctor immediately.
