What is Triapin (Ramipril and Felodipine)?

Triapin 5 mg/5 mg prolonged-release tablets contain two medicines called ramipril and felodipine. Ramipril belongs to a group of medicines called "converting enzyme inhibitors" of angiotensin ”(ACE inhibitors). It works by inhibiting the production of substances that increase blood pressure and relax and dilate blood vessels.

Felodipine belongs to a group of medicines called "calcium antagonists". It acts by relaxing and dilating blood vessels. This helps reduce blood pressure.

How does it work?

Felodipine calcium channel blocker and ACE inhibitor ramipril reduce blood pressure by peripheral vasodilation. Calcium antagonists cause arterial dilation, while ACE inhibitors cause arterial and venous dilation. Vasodilation and therefore reduction of blood pressure can lead to activation of the sympathetic nervous system and the renin-angiotensin system. ACE inhibition leads to a decrease in plasma angiotensin II. The beginning of the Antihypertensive effect of a single dose of Triapin is 1-2 hours. The maximum antihypertensive effect occurs at 2-4 weeks and is maintained during long-term treatment. Tension reduction arterial is maintained during the 24-hour dose interval.

Felodipine is a selective vascular calcium antagonist, which reduces blood pressure by decreasing the peripheral vascular resistance through a direct relaxation action on the vascular smooth muscles. Due to its selectivity for the smooth musculature of the arterioles, felodipine, in therapeutic doses, does not have a direct effect on cardiac contractility or conduction. Felodipine reduces resistance to renal vascular. It does not change the normal glomerular filtration rate. May increase glomerular filtration in patients with impaired renal function.  Felodipine has a moderate natriuretic/diuretic effect, not producing fluid retention.

Ramipril is a prodrug that hydrolyzes giving the active metabolite ramiprilat, a powerful inhibitor of long-acting ACE (angiotensin-converting enzyme). In plasma and tissues, ACE catalyzes the conversion of angiotensin I forming angiotensin II, of vasoconstrictor action, and also causing the splitting of the bradykinin vasodilator. Inhibitor-induced vasodilation ACE causes a reduction in preload and afterload. Since angiotensin II also stimulates release of aldosterone, ramiprilat causes a reduction in aldosterone secretion. The ramipril reduced peripheral arterial resistance without significant changes in renal plasma flow or the rate of glomerular filtration. In hypertensive patients, ramipril causes a reduction in supine blood pressure and standing without a compensatory increase in heart rate.

What are the benefits of taking it?

Triapin is a combined blood pressure medicine (composed of felodipine and ramipril) used in the treatment of essential arterial hypertension. Triapin, fixed-dose combination, is indicated in patients in whom blood pressure is not adequately controlled only with felodipine or only with ramipril.

How do I use it and its dosage?

Always use this medicine exactly as your doctor has told you.  If in doubt, consult your doctor or pharmacist again. Triapin tablets are for adults only. It should not be administered to children. Take this medicine orally.  Take the tablets on an empty stomach or after a light meal.  Swallow the tablets whole with the help of half a glass of water or another drink. You must not leave, crush, or chew the tablets. Do not take your tablets with grapefruit juice or alcohol.

How much to take

The normal dose is 1 Triapin 5 mg / 5 mg tablet once a day.

Your doctor may start your treatment with Triapin 2.5 mg/2.5 mg and then switch to Triapin 5 mg to increase the dose.

The maximum dose is 1 Triapin 5 mg / 5 mg tablet once a day.

If you are taking diuretics (medicines that favor the elimination of water), your doctor may

stop treatment or reduce the diuretic dose before starting treatment with Triapin tablets.

Side effects & precautions

Like all medicines, the Triapin (Ramipril and felodipine) medication can cause side effects, although not everybody gets them.

If you experience any of the following, stop taking Triapin and contact your doctor immediately:

Hypersensitivity and allergic reactions: Signs may include lumps on the skin (hives) or swelling of the face, lips, mouth, tongue, or throat.

The following side effects have been identified. Most of these effects appear at the start of treatment or after a dose increase. If such reactions do occur, they are generally transient and decrease in intensity over time. If you experience any of the following symptoms persistently, please inform your doctor.

Slight swelling of the gums has been reported in patients with inflammation of the mouth (gingivitis/periodontitis). Swelling can be prevented or reversed through careful oral hygiene. Triapin (ramipril and felodipine) side effects include:

Very common: may affect more than 1 in 10 people

  • inflammation of the ankles.

Common: may affect up to 1 in 10 people

  • redness,
  • headache

Uncommon: may affect up to 1 in 100 people

  • abnormally fast heart rate,
  • palpitations,
  • too low blood pressure (hypotension),
  • sickness,
  • abdominal pain,
  • burning / stinging / tingling,
  • rash or itching
  • fatigue,
  • dizziness

Rare: may affect up to 1 in 1,000 people

  • urticaria,
  • Fainting,
  • vomiting
  • joint pain,
  • Muscle pain,
  • impotence / sexual dysfunction.

Very rare: may affect up to 1 in 10,000 people

  • gingivitis (inflamed gums),
  • increased liver enzymes,
  • skin reactions due to hypersensitivity to the sun,
  • inflammation of the blood capillaries of the skin,
  • frequent urination,
  • hypersensitivity reactions such as fever or inflammation of the lips or tongue.
