What is Relvar?

Relvar Ellipta contains two active ingredients: fluticasone furoate and vilanterol. There are two different concentrations of Relvar Ellipta: fluticasone furoate 92 micrograms/vilanterol 22 micrograms and fluticasone furoate 184 micrograms/vilanterol 22 micrograms.

The 92/22 microgram concentration is used for the regular treatment of chronic obstructive pulmonary disease (COPD) in adults, as well as for the treatment of asthma in adults and adolescents 12 years of age and older.

The 184/22 microgram concentration is used for the treatment of asthma in adults and adolescents 12 years of age and older.

The 184/22 microgram concentration is not approved for the treatment of COPD.

How does it work?

Fluticasone furoate and vilanterol are two types of medications (a synthetic corticosteroid and a selective long-acting? 2 receptor agonist).

Fluticasone furoate

Fluticasone furoate is a trifluorinated synthetic corticosteroid with potent anti-inflammatory activity. The exact mechanism by which fluticasone furoate acts on asthma and COPD symptoms is unknown. Corticosteroids have been shown to have a wide variety of actions on various cell types (eg, eosinophils, macrophages, lymphocytes) and mediators (eg, cytokines and chemokines involved in inflammation).

Vilanterol triphenatate

Vilanterol triphenatate is a selective long-acting? 2-adrenergic agonist (LABA).

The pharmacological effect of the active substances? 2-adrenergic agonists, including vilanterol triphenatate, are at least in part attributable to stimulation of intracellular adenylate cyclase, the enzyme that catalyzes the transformation of adenosine triphosphate (ATP) to adenosine monophosphate - 3 ', 5' cyclic (cyclic AMP). The increase in cyclic AMP levels results in relaxation of the bronchial smooth muscle and inhibition of the release of mediators of immediate hypersensitivity of cells, especially mast cells.

Molecular interactions between corticosteroids and LABAs occur, whereby steroids activate the? 2 receptor gene increasing the number of receptors and sensitivity, and LABAs prepare the glucocorticoid receptor for steroid-dependent activation and increase cellular nuclear translocation. These synergistic interactions are reflected in increased anti-inflammatory activity, which has been demonstrated in vitro and in vivo in a variety of inflammatory cells relevant to the pathophysiology of asthma and COPD. In peripheral blood mononuclear cells from subjects with COPD, a greater anti-inflammatory effect was observed in the presence of the combination with fluticasone furoate/vilanterol compared to fluticasone furoate monotherapy at concentrations reached with clinical doses. The potential anti-inflammatory effect of the LABA component was similar to that obtained with other ICS / LABA combinations.

What are the benefits of taking it?

Asthma is a serious chronic lung disease in which the muscles around the smaller airways narrow (bronchoconstriction) and swell and irritate (inflammation). Symptoms come and go and include shortness of breath, wheezing, chest tightness, and cough. Relvar Ellipta benefits include having been shown to reduce asthma attacks and symptoms.

Chronic obstructive pulmonary disease (COPD) is a severe chronic lung disease where the airways become inflamed and thickened. Symptoms include shortness of breath, cough, chest discomfort, and cough accompanied by mucus. Relvar ellipta uses include having been shown to reduce flare-ups of symptoms that accompany COPD.

How do I use it and its dosage?

Always use this medicine exactly as your doctor has told you. If in doubt, ask your doctor, pharmacist, or nurse.

Asthma

The recommended dose for asthma treatment is one inhalation (92 micrograms of fluticasone furoate and 22 micrograms of vilanterol) once a day, administered at the same time each day.

If you have severe asthma, your doctor may decide to take one inhalation from the inhaler that contains the highest concentration (184 micrograms fluticasone furoate and 22 micrograms vilanterol). This dose is also used once a day, at the same time each day.

COPD

The recommended dose for the treatment of COPD is one inhalation (92 micrograms of fluticasone furoate and 22 micrograms of vilanterol) once a day, administered at the same time each day.

The highest concentration of Relvar Ellipta (184 micrograms fluticasone furoate and 22 micrograms vilanterol) is not suitable for the treatment of COPD.

Relvar Ellipta is for inhalation use.

Use Relvar Ellipta at the same time each day as it is effective for 24 hours.

It is very important that you use this medicine every day, as your doctor has recommended. This will help you to have no symptoms during the day or at night.

Relvar Ellipta should not be used to relieve a sudden attack of choking or wheezing. If you have these types of attacks you should use a quick-acting "rescue" inhaler (such as salbutamol).

If you feel you are short of breath or wheezing more often than normal, or if you are using your fast-acting "rescue" inhaler more often than usual, see your doctor.

Instructions for use

Read the following instructions before using the inhaler

If the cap of the inhaler is opened and closed without inhaling the medicine, the dose will be missed. The missed dose will be safely retained within the inhaler, but will not be available for inhalation.

It is not possible to accidentally administer an additional dose or a double dose by inhalation.

How to prepare a dose

Open the cap when you are ready to inhale a dose. Do not shake the inhaler.

Slide the cover down until you hear a 'click'.

The medicine is now ready to be inhaled. As confirmation, the dose counter decreases by 1 unit.

If the dose counter does not decrease upon hearing the 'click', the inhaler will not release the medication. Take it to the pharmacist and ask for help.

How to inhale the medicine

  1. Keep the inhaler away from your mouth and breathe out as reasonably possible.
  2. Do not breathe out into the inhaler.
  3. Place the mouthpiece between your lips and close them tightly around the mouthpiece.
  4. Do not block the ventilation slots with your fingers.
  5. Take a long, continuous, and deep inspiration. Hold your breath as long as possible (at least 3-4 seconds).
  6. Take the inhaler out of your mouth.
  7. Exhale gently and slowly.
  8. You may not be able to tell the taste or notice the medicine, even when you use the inhaler correctly.
  9. If you want to clean the nozzle, use a dry handkerchief before closing the lid.
  10. Close the inhaler and rinse your mouth
  11. Slide the cap up to the stop to protect the mouthpiece.
  12. Rinse mouth with water, once inhaler is used, do not swallow.
  13. This will make side effects such as ulcerations less likely to occur.

Side effects & precautions

Like all medicines, this medicine can cause side effects, although not everybody gets them.

Allergic reactions

Relvar Ellipta rarely causes allergic reactions (may affect up to 1 in 1,000 people)

If you experience any of the following symptoms after taking Relvar Ellipta, stop using this medicine and contact your doctor immediately:

  • skin rash (hives) or redness
  • swelling, sometimes of the face or mouth (angioedema)
  • increased wheezing (sharp sound when breathing), cough, or difficulty breathing
  • A feeling of sudden weakness or dizziness (which can lead to collapse or loss of consciousness).

Very common Relvar ellipta side effects, They can affect more than 1 in 10 people:

  • headache
  • common cold.

Frequent Relvar ellipta side effects, They can affect up to 1 in 10 people:

  • canker sores, bumps in the mouth or throat caused by a fungal infection (yeast infection). Rinsing the mouth with water immediately after using Relvar Ellipta may help prevent this side effect from occurring.
  • inflammation of the lungs (bronchitis)
  • sinus or throat infection
  • flu
  • pain and irritation in the back of the mouth and throat
  • inflammation of the breasts
  • itchy, runny or stuffy nose
  • cough
  • voice disturbances
  • A weakening of the bones which can lead to fractures
  • stomach ache
  • Back pain
  • elevated temperature (fever)
  • joint pain
  • muscle spasms.

Uncommon side effects, They can affect up to 1 in 100 people:

  • irregular heartbeat
  • blurry vision
  • increased blood sugar level (hyperglycemia).

Rare Relvar ellipta side effects, They can affect up to 1 in 1,000 people

  • fast heartbeat (tachycardia)
  • notice your heartbeat (palpitations)
  • tremor
  • anxiety
