Lercanidipine Tablets

Lercanidipine is a medicine classified as a calcium channel blocker: medicines that dilate blood vessels and lower blood pressure. Doctors prescribe lercanidipine to treat high blood pressure when other antihypertensive medications do not help (well enough). Lercanidipine should be taken at a fixed time (preferably in the morning) and always at least 15 minutes before eating. Note: Grapefruit may improve the effects of these medications, so you should not eat/drink grapefruit (juice) if you are being treated with this medication. Lercanidipine is available in 10 and 20 mg tablets. The doctor will know what dose is right for you. The usual dose for adults over 18 years is one tablet once a day. Always follow the doctor's advice. You can still take a missed dose unless it is almost time for the next dose. About Lercanidipine Tablets

Lercanidipine is a calcium channel blocker used to treat hypertension (high blood pressure). It widens the blood vessels, which decreases the pressure on the heart. It is a daily tablet available in two strengths: 10mg and 20mg. Lercanidipine may take up to two weeks to reach its full effect, but it has a longer duration than other similar medicines. Lercanidipine is popular because, for some people, it is more easily tolerated than other blood pressure medicines.

Calcium channel blockers are used to treat mild to moderate cases of hypertension. Lercanidipine works by targeting muscle cells on the arterial walls throughout the body. These muscle cells require calcium to contract, and calcium channel blockers actively reduce their access to calcium. This means instead of contracting, the arterial muscles start to relax. This results in a mild widening of the arteries, which makes it easier for the heart to circulate blood.

What is Lercanidipine?

Lercanidipine, lercanidipine hydrochloride, belongs to a group of medicines called calcium entry blockers (dihydropyridine derivatives) that lower blood pressure. Lercanidipine is used to treat high blood pressure, also known as hypertension in adults over 18 years of age (it is not recommended to children and adolescents under 18).

How does it work?

The benefits of Lercanidipine are due to it being a calcium antagonist from the dihydropyridine group that inhibits the influx of calcium through the membrane into cardiac and smooth muscle. The mechanism of its antihypertensive effect is due to a direct relaxing effect on vascular smooth muscle, thus reducing total peripheral resistance. Despite its short half-life, lercanidipine is endowed with prolonged antihypertensive activity since its membrane partition coefficient is high, and it lacks negative inotropic effects due to its high vascular selectivity.

Since lercanidipine-induced vasodilation has a gradual onset, acute hypotension with reflex tachycardia has rarely been observed in hypertensive patients. Like other 1-4 asymmetric dihydropyridines, the antihypertensive activity of lercanidipine is primarily due to its S enantiomer.

How do I use it and its dosage?

Adults: The recommended dose is one daily tablet of lercanidipine (corresponding to 10 mg of lercanidipine hydrochloride), at the same time each day, preferably in the morning, at least 15 minutes before breakfast, because a high-fat meal significantly increases the blood concentration of the active ingredient. If necessary, your doctor may advise you to increase the dose to two lercanidipine tablets daily (corresponding to 20 mg of lercanidipine hydrochloride).

The tablet should be taken with a sufficient amount of liquid (for example, a glass of water).

Elderly: No daily dose adjustment is required. However, special care should be taken when starting treatment.

Patients with kidney or liver problems: Special care is needed when starting treatment in these patients and an increase in the daily dose to 20 mg should be done with caution. The effect of lowering blood pressure in patients with liver problems may be greater and as a consequence, a dose adjustment should be considered.

Side effects & precautions

If you experience any of these side effects, tell your doctor immediately.

Rare: angina pectoris (chest pain due to lack of blood supply to the heart).

Very rare: chest pain, decreased blood pressure, dizziness, and allergic reactions (symptoms consist of itching, rash, hives).

If you have previously had an attack of angina pectoris, treatment with medicines from the group to which Lercanidipine belongs may cause an increase in the frequency, duration, or severity of these attacks. Isolated cases of myocardial infarction can be observed.

Other possible side effects:

Uncommon: headache, dizziness, tachycardia, palpitations (increased frequency of heartbeat), sudden redness of the face, neck or upper face, swelling of the ankles.

Rare: drowsiness, nausea, vomiting, burning chest, stomach pain, diarrhea; skin rash, muscle pain, elimination of large amounts of urine, tiredness.

Very rare: swelling of the gums, changes in liver function (detected by blood tests), increased number of times you urinate.

Sick sinus syndrome

Special care should be taken when lercanidipine is used in patients with sick sinus syndrome (if a pacemaker is not in place). Although controlled hemodynamic studies revealed the absence of disturbances in ventricular function, caution should also be exercised in patients with left ventricular dysfunction. This has suggested that some short half-life dihydropyridines may be associated with increased cardiovascular risk in patients with ischemic heart disease. Although lercanidipine is long-lasting, caution should also be exercised with this type of patient.

Angina pectoris

Some dihydropyridines rare can cause chest pain or angina. Very rarely, patients with pre-existing angina pectoris may experience increased frequency, duration, or severity of these attacks. Isolated cases of myocardial infarction can be observed.

Use in patients with kidney and liver failure:

Special care should be taken when starting treatment in patients with mild to moderate impairment of liver function. Although the usually recommended dosage regimen can be tolerated by these population subgroups, caution should be exercised when increasing the dose to 20 mg daily. Dose adjustment should be considered in patients with hepatic impairment, since the antihypertensive effect may be increased.
