Maxidex is indicated for the treatment of non-infectious inflammations of the eyes that respond to corticosteroids, such as anterior uveitis (inflammation of the uvea or middle layer of the eye), inflammation of the iris (iritis), of the ciliary body (cyclitis) or joint inflammations (iridocyclitis). Inflammations of the cornea (keratitis): keratitis caused by the Herpes Zoster virus, superficial punctate keratitis (with small points of injury to the cornea). Complement in the treatment of phlyctenular keratoconjunctivitis (inflammation of the cornea and conjunctiva characterized by the presence of small nodules). Spring and allergic conjunctivitis. Foreign body injuries of the cornea.

Dexamethasone has a long duration of action. Corticosteroids have multiple actions that produce anti-inflammatory and immunosuppressive effects. They decrease inflammation by stabilizing lysosomes in neutrophils. Research additionally demonstrated that corticosteroids also induce the anti-inflammatory protein lipocortin, which inhibits the enzyme phospholipase A2 that inhibits the synthesis of prostaglandins and products of the lipoxygenase. Corticosteroids also bind to glucocorticoid receptors (GRs) located in the cytoplasm. After binding, the activated GR goes to the nucleus where upregulation of anti-inflammatory genes (eg, lipocortin, neutral endopeptidase, plasminogen activator inhibitors).
Maxidex 1 mg/ml eye drops, the suspension is indicated in the treatment of inflammatory manifestations. Maxidex uses include being utilised in the treatment of the following symptoms:
- Inflammatory processes of the anterior segment of the eye: anterior uveitis, iritis, iridocyclitis, cyclitis.
- External diseases: superficial punctate keratitis, Herpes Zoster keratitis, adjuvant
- In phlyctenular keratoconjunctivitis, conjunctivitis (spring and allergic).
- Foreign body injuries of the cornea.
- Thermal burns.
Always use this medicine exactly as your doctor has told you. If in doubt, consult your doctor or pharmacist again. The recommended dose is:
Adults:
In general instill 1 or 2 drops in the affected eye, 5 or 6 times a day.
When a satisfactory response is observed at 3-4 days, the frequency of administration can be gradually reduced to fewer times a day.

Recommendations for use:
- Wash your hands.
- Take the bottle (dropper container) and shake it well before use.
- After opening the bottle for the first time, you must remove the plastic ring from the seal if it is loose.
- Hold the bottle upside down.
- Tilt your head back. Gently separate the eyelid from the eye with a finger until a pocket forms between the eyelid and your eye, into which the drop should fall.
- Bring the tip of the bottle close to the eye. The mirror may be helpful.
- Do not touch the eye or eyelid, proximal areas, or other surfaces with the dropper. The drops could become contaminated.
- Gently squeeze the base of the bottle so that it drops one drop at a time.
- After using this eye drop, press the edge of the eye next to the nose with your finger. This helps prevent this medication from passing to the rest of the body.
- If drops are applied to both eyes, repeat all of the above steps with the other eye.
- Close the bottle tightly immediately after using the product.
If a drop falls out of the eye, try again.
Like all medicines, this medicine can cause side effects, although not everybody gets them. The following Maxidex side effects have been reported with this medicine:

- inflammation of the surface of the eye (keratitis), inflammation of the conjunctiva (conjunctivitis), dry eye, spot on the cornea, sensitivity to light (photophobia), blurred vision, abnormal sensation in the eye, tearing, crusting of the eyelid, itching, irritation or redness of the eye.
General effects:
- alteration of the sense of taste (bad taste).
Effects in the eye:
- glaucoma (increased eye pressure with decreased visual acuity), corneal ulcer, increased pressure in the eye (s), reduced vision, corneal damage, drooping eyelid, pain in the eye, increase in the size of the pupil.
General effects:
- hypersensitivity (allergy), dizziness, headache.
Hormonal problems:
- excessive growth of body hair (particularly in women), muscle weakness and wasting, purple striations on the skin of the body, increased blood pressure, irregular or absent menses, changes in the body's protein and calcium levels, growth retardation in children and adolescents and swelling and weight gain of the body and face (Cushing's syndrome).
Long-term use of corticosteroids in the eyes can also cause:
- Increased eye pressure with damage to the optic nerve and decreased visual acuity.
- Cataract formation.
- Delayed healing of the cornea.
- diseases that cause thinning of the cornea, there is an increased risk of perforation.
Maxidex interactions.
Concomitant use of steroids and ophthalmic NSAIDs can increase the potential for corneal scarring problems.
CYP3A4 inhibitors (ritonavir and cobicistat) may decrease dexamethasone clearance resulting in increased effects and Cushing's syndrome / adrenal suppression. This association should be avoided unless the benefit outweighs the increased risk of systemic corticosteroid adverse reactions, in which case patients should be monitored for systemic corticosteroid reactions.
