What is DuoResp Spiromax?
DuoResp Spiromax contains two different active ingredients: budesonide and formoterol fumarate dihydrate.
Budesonide belongs to a group of medicines called "corticosteroids" also known as "steroids". It works by reducing and preventing swelling and inflammation of the lungs and helping you breathe easier.
Formoterol fumarate dihydrate belongs to a group of medicines called "long-acting β2 adrenergic receptor agonists" or "bronchodilators". It works by relaxing the muscles in the airways. This will help open up the airways and help you breathe easier.
How does DuoResp Spiromax work?
DuoResp Spiromax contains formoterol and budesonide, which have different modes of action and show additive effects in terms of reducing asthma exacerbations. The specific properties of budesonide and formoterol allow the combination to be used as maintenance and relief therapy, or as maintenance therapy for asthma. The mechanisms of action of the two pharmaceutical substances respectively are discussed below.
Budesonide
Budesonide is a glucocorticosteroid that, by inhalation use, exerts a dose-dependent anti-inflammatory action in the airways, reducing asthma symptoms and exacerbations. Inhaled budesonide causes fewer serious adverse reactions than systemic corticosteroids. The exact mechanism responsible for the anti-inflammatory effect of glucocorticosteroids is unknown.
Formoterol
Formoterol is a selective β2-adrenergic receptor agonist whose inhalation causes rapid, long-acting relaxation of the bronchial smooth muscle in patients with reversible airway obstruction. The bronchodilator effect depends on the dose and takes between 1 and 3 minutes to occur. The duration of the effect is at least 12 hours after administering a single dose.
What are the benefits of using DuoResp Spiromax?
DuoResp Spiromax is indicated for use only in adults 18 years of age and older. DuoResp Spiromax is NOT indicated for use in children under 12 years of age or adolescents between 13 and 17 years of age.
Your doctor has prescribed this medicine as a treatment for asthma or chronic obstructive pulmonary disease (COPD).
Asthma
If used to treat asthma, your doctor will prescribe DuoResp Spiromax along with an additional "relief inhaler" such as salbutamol.
Use DuoResp Spiromax every day. This helps prevent asthma symptoms such as shortness of breath and wheezing (chest beeping) from appearing.
Use the "reliever inhaler" when you have asthma symptoms to breathe easier.
Do not use DuoResp Spiromax 320/9 micrograms as a "reliever inhaler".
Chronic obstructive pulmonary disease (COPD)
COPD is a long-lasting disease that affects the airways of the lungs, often caused by cigarette smoking. Some of its symptoms are shortness of breath, cough, chest discomfort, and expectoration. DuoResp Spiromax can also be used to treat symptoms of severe COPD in adults.
How do I use DuoResp Spiromax and its dosage?
Always use the duoresp spiromax BNF inhaler exactly as your doctor or pharmacist has told you. If in doubt, ask your doctor, pharmacist, or nurse again.
It is important to use DuoResp Spiromax daily, even if you do not have asthma or COPD symptoms at that time. If you are using DuoResp Spiromax for asthma, your doctor will want to check your symptoms regularly. Use DuoResp Spiromax every day. This helps prevent asthma symptoms from appearing.
Recommended dose:
Adults (18 years and older)
1 inhalation (application) twice a day, administered in the morning and at night.
Your doctor may increase the dose up to 2 inhalations twice a day.
If your symptoms are well controlled, your doctor may ask you to use this medicine once a day.
The doctor will help you control your asthma and adjust the dose of this medicine to the lowest possible to control your asthma. If your doctor thinks that you need a lower dose than is available in your DuoResp Spiromax, your doctor may prescribe an alternative inhaler that contains the same active ingredients as your DuoResp Spiromax, but with a lower dose of the corticosteroid. However, do not adjust the number of inhalations prescribed by your doctor without first talking to him.
Use the other "reliever inhaler" to treat asthma symptoms when they appear.
Always carry the “relief inhaler” with you and use it to relieve sudden attacks of shortness of breath and wheezing (wheezing). Do not use DuoResp Spiromax to treat these asthma symptoms.
Chronic obstructive pulmonary disease (COPD)
Recommended dose:
Adults (18 years and older):
1 inhalation twice a day, administered in the morning and at night.
Your doctor may also prescribe another bronchodilator medicine, such as an anticholinergic (such as tiotropium or ipratropium bromide), for your COPD.
How to use this Chronic Obstructive Pulmonary Disease medication.
Every time you need to do an inhalation, follow the instructions below:
- Hold the inhaler with the wine red semi-transparent mouthpiece cap at the bottom
- Open the mouthpiece cover by folding it down until a loud click is heard. The drug is actively measured. The inhaler is ready for use. Expel all the air gently (without being uncomfortable). Do not expel air through the inhaler.
- Put the mouthpiece between your teeth. Do not bite the mouthpiece. Close your lips around the mouthpiece. Be careful not to block the air inlets. Breathe in as deeply and strongly as you can through your mouth.
- Remove the inhaler from your mouth. You may notice some flavor when inhaling.
- Hold your breath for 10 seconds or as long as you can without being uncomfortable.
- Next, blow the air out gently (but without blowing the air out through the inhaler). Close the mouthpiece cover.
If you are going to do a second inhalation, repeat steps 1-7.
Rinse your mouth with water after each dose and spit it out. Do not attempt to disassemble the inhaler or to remove or twist the mouthpiece cap, as it is attached to the inhaler and should not come off. Do not use the Spiromax if it is damaged or if the mouthpiece has come off the Spiromax. Do not open or close the mouthpiece cap unless you are going to use the inhaler.
What are the Side effects & precautions of DuoResp Spiromax?
Like all medicines, this medicine can cause side effects, although not everybody gets them.
If you notice any of the following symptoms, stop using DuoResp Spiromax and contact your doctor immediately:
Rare side effects: may affect up to 1 in 1,000 people
- Swelling of the face, especially around the mouth (tongue and/or throat, and/or difficulty swallowing) or hives along with shortness of breath (angioedema) and/or a sudden feeling of fainting. This may indicate that you are having an allergic reaction, which may also include a rash and itching.
- Bronchospasm (tightness of the muscles of the airways causing whistling and shortness of breath). If beeping suddenly appears after using this medicine, stop using it, and see a doctor immediately.
Very rare side effects: may affect up to 1 in 10,000 people
- Sudden, unexpected, sharp wheezing (beeping) and / or shortness of breath immediately after using the inhaler (also called “paradoxical bronchospasm”). If any of these symptoms occur, immediately stop using DuoResp Spiromax and use the "reliever inhaler", if you have one. See a doctor immediately, as you may need to change treatment.
Other possible Duresp Spiromax side effects:
Common: may affect up to 1 in 10 people
- Palpitations (awareness of your own heartbeat), tremors, or shaking. If these effects occur, they will generally be mild and will disappear as you continue to use DuoResp Spiromax.
- Candida (a fungal infection) in the mouth. This will be less likely to occur if you rinse your mouth with water after using the medicine.
- Mild sore throat, cough, and hoarseness.
- Headache
- Pneumonia (infection of the lungs) in patients with COPD.
Tell your doctor if you have any of the following symptoms while inhaling DuoResp Spiromax, they could be symptoms of a lung infection:
- Fever or chills
- Increased mucus production, change in mucus color
- Increased cough or increased difficulty breathing
Uncommon: may affect up to 1 in 100 people
- Feeling restless, nervous, agitated, anxious, or angry.
- Trouble sleeping
- Feeling dizzy
- Nausea (upset stomach).
- Fast heartbeat.
- Bruises
- Muscle cramps.
- Blurry vision
Rare:
- Low levels of potassium in the blood.
- Irregular heartbeat.
Very rare:
- Depression
- Changes in behavior, especially in children.
- Pain or tightness in the chest (angina pectoris).
- Alteration of the heart's electrical system that does not cause symptoms (prolongation of the QTc interval).
- An increased amount of sugar (glucose) in the blood when having a blood test.
- Alterations in the perception of taste, such as an unpleasant taste in the mouth.
- Changes in blood pressure.
- Inhaled corticosteroids can affect the normal production of steroid hormones in the body, especially if they are used in high doses for long periods. Among its effects are:
- changes in bone mineral density (weakening of the bones)
- cataracts (clouding of the lens of the eye)
- glaucoma (increased pressure in the eye)
- slowing down the growth rate of children and adolescents
- effects on the adrenal gland (a small gland next to the kidney). Symptoms of inhibition of the adrenal gland could be tiredness, weakness, upset stomach including nausea, vomiting, pain and diarrhea, darkening of the skin, and weight loss.
