Cosopt is a beta-blocker eye drop medication that contains two different main actives medicines: dorzolamide and timolol. Dorzolamide belongs to a group of medicines called carbonic anhydrase inhibitors. Timolol belongs to a group of medicines called beta-blockers. These drugs lower intraocular pressure in different ways. What is Cosopt?
Cosopt is indicated for the treatment of elevated intraocular pressure (IOP) in patients with open-angle glaucoma, or pseudoexfoliative glaucoma when monotherapy with a beta-blocker is not enough.
How does it work?
Cosopt eye drops solution is comprised of two main active ingredients, Dorzolamide and Timolol, each of these two components lower elevated intraocular pressure, reducing the secretion of aqueous humor, although each using different mechanisms of action. Dorzolamide hydrochloride is a potent inhibitor of human carbonic anhydrase II. Inhibition of carbonic anhydrase in the ciliary processes of the eyes decreases the secretion of aqueous humor, probably slowing down the formation of bicarbonate ions with the subsequent reduction of sodium and fluid transport. Timolol maleate is a non-selective blocking agent for beta-adrenergic receptors. For now, the exact mechanism of action is not clearly established.

The main benefit this eye drop solution is associated with is being used to reduce high intraocular pressure (ocular hypertension) and in the treatment of glaucoma (open-angle glaucoma, pseudoexfoliative glaucoma). How do I use it and its dosage?
Always use this medicine exactly as your doctor has told you. If in doubt, consult your doctor or pharmacist again. The appropriate dosage and duration of treatment will be established by your doctor.
Wash your hands.
Remove a strip of packaging from the envelope.
Detach a single-dose container from the strip.
Put the remaining strips back into the envelope and fold the edge of the envelope to close it.
To open the package, turn the tab.
Hold the container between your thumb and index finger. Please note that the tip of the container should not protrude more than 5 mm above your index finger.
Tilt your head back or lie down. Place your hand on your forehead. Your index finger should be aligned with your eyebrow or rest on the bridge of your nose. Look up. Pull the lower eyelid down with the other hand. Do not let any part of the container touch your eye or any area around your eye. Gently squeeze the container to drop a drop into the space between the eyelid and the eye. Do not blink while applying the drop to your eye. Each single-dose container contains enough solution for both eyes.
Close your eye and press the inside corner of your eye with your finger for about two minutes. This helps stop the entry of the medication into the rest of the body.
Wipe any excess solution from the skin around the eye.
The recommended dose is one drop in the affected eye or eyes in the morning and at night.
If you use this medicine at the same time as another eye drop, the drops should be applied at least 10 minutes apart. Do not change the dose of the medicine without consulting the doctor. Do not let the tip of the container touch your eyes or the areas around them. It can be contaminated with bacteria capable of causing eye infections that cause serious damage to the eyes, and even loss of vision. To avoid possible contamination of the container, wash your hands before using this medicine, and avoid contacting the tip of the container with any surface. If you think your medicine may be contaminated, or if you develop an eye infection, see your doctor immediately to see if you can continue to use this package.
Side effects & precautions
Like all medicines, this medicine can cause side effects, although not everybody gets them. You can usually continue using the drops unless the effects are severe. If you are concerned, ask your doctor or pharmacist. Do not stop using Cosopt without talking to your doctor. Generalized reactions including swelling under the skin that can occur in areas such
as the face and limbs, and can obstruct the airways, causing difficulty swallowing or breathing, hives or an itchy rash, generalized and localized rash, itching, allergic reaction serious that suddenly puts life in danger.
The following adverse reactions have been reported with Cosopt or one of its components during clinical trials or after marketing:

Eye pain, dry eyes
Blurred vision
Headache
Sinusitis (a feeling of tightness or stuffy nose)

Weakness/tiredness
Fatigue

Dizziness
Depression
Inflammation of the iris
Vision disturbances
Including changes in refraction (in some cases due to discontinuation of miotic therapy)
