Sotalol is a beta-blocker that affects the heart and circulation within the atrium and ventricles (the upper and lower chambers of the heart that allow blood to flow in and out of the heart). Sotalol is used to help keep the heart beating normally in people with certain heart rhythm disorders, such as atrial fibrillation, atrial flutter, ventricular tachycardia, and ventricular fibrillation. Usually, the starting dose is one tablet (80 mg). The dose of Sotalol should be adjusted gradually, at intervals of 2-3 days between each dose increase, so it will be necessary to visit your doctor regularly during the first weeks after starting treatment with Sotalol, to check your response and if consider it necessary, adjust the dose.

About Sotalol Tablets

Sotalol is used to treat irregular heartbeats. Sotalol is in a class of medications called antiarrhythmics. It works by acting on the heart muscle to improve the heart's rhythm. Sotalol (Betapace) is usually taken twice a day and sotalol (Betapace AF) is usually taken once or twice a day. Take sotalol consistently, either with food or without food each time. Follow the directions on your prescription label carefully, and ask your doctor or pharmacist to explain any part you do not understand. Take sotalol exactly as directed. Do not take more or less of it or take it more often than prescribed by your doctor.

Sotalol controls your condition but does not cure it.

What is Sotalol?

Sotalol is a medicine that contains an active ingredient called sotalol that belongs to a group of medicines called beta-adrenergic blocking agents or beta-blockers.

How does it work?

D, l-sotalol is nonselective Adrenergic receptor blocking hydrophilic agents, lacking intrinsic sympathomimetic activity and membrane-stabilizing activity.

Sotalol has two antiarrhythmic properties, as a β-adrenoreceptor blocker (Vaughan Williams Class II) and by prolonging the duration of cardiac action potential (Vaughan Williams Class III). Sotalol has no known effect on the ascending velocity and therefore has no effect on the depolarization phase.

Sotalol uniformly prolongs the duration of the action potential in cardiac tissues by delaying the repolarization phase. Its main effects are the prolongation of the effective refractory periods of the atrial and ventricular accessory routes.

Class II and III properties can be reflected on the surface electrocardiogram by prolonging the PR, QT, and QTC intervals (QT corrected for heart rate) without significant changes in QRS duration.

The d- and l- isomers of sotalol HCL have similar Class III antiarrhythmic effects while the l- isomer is virtually responsible for all Blocking activity. Although significant Blocking effects may appear at oral doses as low as 25 mg, Class III effects are generally seen at daily doses above 160 mg.

Its blocking activity of Adrenergic receptors produces a reduction in heart rate (negative chronotropic effect) and a limited reduction in contractility (negative inotropic effect). These cardiac modifications reduce myocardial oxygen consumption and cardiac work. As with other Blockers, sotalol inhibits renin release. This suppressive effect of renin is significant, both at rest and during exercise. Like other Adrenergic blockers, Sotalol produces a gradual but significant reduction in blood pressure, systolic and diastolic, in hypertensive patients. With a single daily dose, blood pressure control is maintained for twenty-four hours in both the supine and standing positions.

Ventricular arrhythmias:

  • Treatment of life-threatening ventricular tachyarrhythmias.
  • Treatment of symptomatic unsupported ventricular tachyarrhythmias.

Supraventricular arrhythmias:

  • Prophylaxis of paroxysmal atrial tachycardia, paroxysmal atrial fibrillation, paroxysmal A-V nodal reentry tachycardia, paroxysmal A-V reentry tachycardia incorporating accessory pathways and paroxysmal supraventricular tachycardia after cardiac surgery.
  • Maintenance of sinus rhythm after conversion of atrial fibrillation or atrial flutter

How do I use it and its dosage?

Always use this blood pressure medicine as your doctor has told you. If in doubt, ask your doctor or pharmacist.

The Sotalol Dosage can be taken in a single dose or in divided doses.

Usually, the starting dose is one tablet (80 mg). The dose of Sotalol should be adjusted gradually, at intervals of 2-3 days between each dose increase, so it will be necessary to visit your doctor regularly during the first weeks after starting treatment with Sotalol, to check your response and if consider it necessary, adjust the dose.

Most patients respond to a daily dose of 160-320 mg/day, administered in 2 divided doses at 12-hour intervals.

Some patients with life-threatening refractory ventricular arrhythmias may require doses of 480-640 mg/day.

You should not stop Sotalol treatment abruptly.

Use in children and adolescents

The efficacy and safety of Sotalol in children under 18 years of age have not been established.

Use in elderly patients

In elderly patients with impaired renal function and other organic disturbances, lower doses are usually used.

Renal insufficiency

In patients with renal disorders, lower doses are usually used.

Side effects & precautions

Like all medicines, Sotalol side effects can occur, although not everyone gets them. The following side effects have been observed:

Very common (may affect more than 1 in 10 people):

  • bradycardia (slow heart rate), chest pain, palpitations, edema (swelling), abnormalities on the electrocardiogram, dyspnea (shortness of breath), vomiting, diarrhea, dizziness, asthenia (weakness), lightheadedness, headache, sleep disturbances.

Common (may affect up to 1 in 10 people):

  • arrhythmia (heart rhythm disorders), syncope (fainting), heart failure, presyncope (lipothymia), torsades de pointes (a type of arrhythmia), asthma, rash skin, itching (itching), alopecia, hyperhidrosis (excessive sweating), nausea, dyspepsia (upset digestion), stomach pain, gas, dysgeusia (taste disorders), muscle cramps, myalgia (muscle aches), arthralgia (pain joints), fatigue, sleep disorders, tingling, depression, anxiety, mood swings, sexual dysfunction, vision disorders, hearing disorders, low blood pressure, fever and hearing impairment.

Special warnings and precautions

Abrupt withdrawal:

Never stop treatment abruptly in patients with angina pectoris. Hypersensitivity to catecholamines has been observed in patients who discontinued treatment with blockers. Occasional cases of exacerbation of angina pectoris, severe arrhythmias, and in some cases, myocardial infarction have been reported after abrupt withdrawal of treatment. Patients should be monitored when chronic Sotapor treatment is withdrawn, especially those with ischemic heart disease.

The dose should be gradually reduced over a period of one to two weeks, and if necessary, start the replacement treatment at the same time. Sudden discontinuation can unmask latent coronary failure. Also, hypertension can develop.
