Bendroflumethiazide

Bendroflumethiazide is a diuretic of the thiazide family and as such acts on the renal tubular mechanism of electrolyte absorption. It inhibits sodium reabsorption in the distal nephron, increases urinary excretion of sodium and chloride by approximately equal levels, and also increases urinary excretion of potassium. It is used as an antihypertensive agent, without the mechanism of action being explained yet. Bendroflumethiazide does not affect normal blood pressure but lowers elevated values. In chronic administration, some patients develop hypokalemia, hyperuricemia, and hyperglycemia; Hyponatremia and hypochloremia may occasionally be observed in edematous patients. Bicarbonate excretion is slightly increased by bendroflumethiazide, but not enough to cause a change in urinary pH. It is absorbed relatively quickly after oral administration. Its normal half-life is 15 hours, the diuretic effect begins at 2 hours, the maximum effect is obtained at 4 hours and its action lasts from 6 to 12 hours.

About Bendroflumethiazide Tablets

Bendroflumethiazide is a thiazide diuretic, a type of medicine more commonly known as ‘water tablets’. This 2.5mg daily tablet is a popular treatment for high blood pressure and fluid retention (oedema). As the name suggests, water tablets aid in the drainage of water from the blood and other parts of the body. This reduces pressure on the heart and treats other complications that can occur due to oedema.

What is Bendroflumethiazide?

Bendroflumethiazide is an orally active thiazide diuretic, indicated for the treatment of hypertension, as an adjunct to edema associated with congestive heart failure, liver cirrhosis, and treatment with corticosteroids and estrogens. It is also used in the treatment of hypercalciuria and diabetes insipidus.

How does it work?

Thiazide diuretics increase the excretion of sodium, chlorides, and water by inhibiting the transport of sodium ion through the renal tubular epithelium. Although thiazides have more than one effect, the main mechanism responsible for diuresis is the inhibition of chloride reabsorption in the distal portion of the ascending loop, that is, in the first section of the distal tubule. It is not known exactly how chlorine transport is inhibited. By increasing the sodium load in the renal tubule, thiazide diuretics indirectly increase potassium excretion through a sodium-potassium exchange mechanism. Thiazides also increase the elimination of bicarbonate and reduce the elimination of calcium and uric acid.

Bendroflumethiazide is a thiazide diuretic that works by inhibiting sodium reabsorption at the beginning of the distal convoluted tubule (DCT). Water is lost as a result of more sodium reaching the collecting ducts. Bendroflumethiazide has a role in the treatment of mild heart failure, although loop diuretics are better at reducing overload. The main use of bendroflumethiazide is currently in hypertension (part of the effect is due to vasodilation).

How do I use it?

Take this medication by mouth with a glass of water. Follow the directions on the medicine label. This medicine can be taken with or without food. Take your doses at regular intervals. Do not take your medicine more often than directed. Do not stop taking it unless your doctor or healthcare professional tells you to. Its dosage

The recommended bendroflumethiazide 2.5 mg tablet dose treatment of hypertension:

Oral administration:

Adults: Initially a dose of 5-20 mg once a day. Maintenance doses can be 2.5-15 mg once a day

Children: up to 0.4 mg/kg or 12 mg / m2 daily in a single dose or divided into two administrations

The recommended bendroflumethiazide 2.5 mg tablet dose treatment of edema or diabetes insipidus:

Oral administration:

Adults: Initially 5 to 10 mg once a day. Maintenance doses will be 2.5-5 mg once a day

Children: Between 0.05 and 0.4 mg / kg / day in one or two administrations. Maintenance doses will be 0.1 to 0.3 mg / kg / day (1.5 to 3 mg / m2)

In patients with moderate renal impairment (creatinine clearance> 30 ml/min) no dose adjustment is required. In patients with more severe renal impairment, the use of bendroflumethiazide is not recommended

Interactions

Diazoxide: additive hyperglycemic activity.

Digitalis: Hypokalemia induced by diuretics as well as hypomagnesemia can precipitate the toxicity of digitalis

Lithium: Thiazide diuretics can increase lithium toxicidac by reducing your kidney excretion

Sulfonylureas, insulin: Thiazide diuretics increase fasting glucose levels. Readjustments in insulin or oral antidiabetic doses may be necessary.

Amphotericin B, corticosteroids: May cause additive electrolyte depletion, particularly hypokalemia

Bile acid sequestrants: can bind thiazides and reduce their absorption.

NSAIDs: Nonsteroidal anti-inflammatory drugs can reduce the diuretic, natriuretic, and antihypertensive effects of thiazides.

Precautions and warnings.

The risk of hypokalemia is high in those who receive bendroflumethiazide; if it develops, it may be necessary to supplement the diet with potassium. Other signs of electrolyte imbalance should be noted. It can increase blood glucose in diabetics and appear photosensitivity. Hypotension and hypokalemia are more frequent in the elderly. Bendroflumethiazide crosses the placenta and may cause non-teratogenic adverse effects (fetal or neonatal jaundice, thrombocytopenia, and effects similar to those seen in adults) on the fetus, therefore the risk-benefit ratio should be evaluated in pregnant women; Furthermore, in the absence of conclusive evidence of teratogenesis, it is recommended not to use in these women unless the benefit to the mother outweighs the potential risk to the fetus. Breastfeeding will be suspended. The safety and effectiveness in children have not been established.
